When you make the choice to choose the best, requesting our services is as simple as calling our office – 937-548-0506. WE WILL TAKE IT FROM THERE!
You are welcome to stop in our office anytime from 7:00 A.M. – 5:00 P.M. Monday through Friday and meet with one of our staff to discuss services.
We are also available to come to your hospital room, nursing home, or your home.
CHN will provide patient-centered, quality care in your home, apartment or even if you live in an assisted living facility.
If you are being discharged from a hospital, the discharge planner will work with you to arrange for home care. The hospital is required to give you a list of all home care agencies available in your area. You have the right to choose your home care provider.
